If an OTP device is misprogrammed, it must be physically replaced.

The MCLR/Vpp pin (pin 4) requires a 10 kohm pull-up to VDD for normal operation. During ICSP (in-circuit serial programming) the MCLR line must be driven to Vpp (typically 13.4V for OTP EPROM parts) - use a diode-isolated programming connector.
